Pull DMA mapping updates from Christoph Hellwig:

 - remove the already broken support for NULL dev arguments to the DMA
   API calls

 - Kconfig tidyups

Most dma_map_ops implementations already had some issues with a NULL
device, or did simply crash if one was fed to them.  Now that we have
cleaned up all the obvious offenders we can stop to pretend we
support this mode.

We don't require drivers to guess a DMA mask that might actually
match the system capabilities any more, so fix up the documentation
to clear this up.

Each text file under Documentation follows a different
format. Some doesn't even have titles!

Change its representation to follow the adopted standard,
using ReST markups for it to be parseable by Sphinx:

- Mark titles;
- Mark literal blocks;
- Mark some literals that would otherwise produce warnings;
- Mark authorship.

Move the generic implementation to &lt;linux/dma-mapping.h&gt; now that all
architectures support it and remove the HAVE_DMA_ATTR Kconfig symbol now
that everyone supports them.

The nents argument to the DMA API functions operating on scatterlists is
always the same. The documentation used different argument names and the
matter was not mentioned in Documentation/DMA-API-HOWTO.txt at all. Fix
these.
