commit db0fa0cb0157 "scatterlist: use sg_phys()" did replacements of the form: phys_addr_t phys = page_to_phys(sg_page(s)); phys_addr_t phys = sg_phys(s) & PAGE_MASK; However, this breaks platforms where sizeof(phys_addr_t) > sizeof(unsigned long). Revert for 4.3 and 4.4 to make room for a combined helper in 4.5. microblaze/timer: Migrate to new 'set-state' interface

Migrate microblaze driver to the new 'set-state' interface provided by clockevents core, the earlier 'set-mode' interface is marked obsolete now. This also enables us to implement callbacks for new states of clockevent devices, for example: ONESHOT_STOPPED. The IRQCHIP_DECLARE macro migrated to 'include/linux/irqchip.h'. See commit 91e20b5040c67c51aad88cf87db4305c5bd7f79d ("irqchip: Move IRQCHIP_DECLARE macro to include/linux/irqchip.h"). This patch removes the inclusion of private header 'drivers/irqchip/irqchip.h' and replaces it with the inclusion of 'include/linux/irqchip.h'.
