powerpc: iommu: fix build when neither PCI or IBMVIO is set

[ Upstream commit b27dadecdf9102838331b9a0b41ffc1cfe288154 ] When neither CONFIG_PCI nor CONFIG_IBMVIO is set/enabled, iommu.c has a build error. The fault injection code is not useful in that kernel config, so make the FAIL_IOMMU option depend on PCI || IBMVIO. powerpc/mm: Catch usage of cpu/mmu_has_feature() before jump label init

This allows us to catch incorrect usage of cpu_has_feature() and mmu_has_feature() prior to jump labels being initialised. mpe: Use printk() and dump_stack() rather than WARN_ON(), because WARN_ON() may not work this early in boot. The pseries HVC early debug options, CONFIG_PPC_EARLY_DEBUG_LPAR and CONFIG_PPC_EARLY_DEBUG_LPAR_HVSI both require code that is part of the hvc driver. If we turn them on but not CONFIG_HVC_CONSOLE then we get: arch/powerpc/kernel/built-in.o: In function `.udbg_early_init': arch/powerpc/kernel/built-in.o:(.debug_addr+0x9a00): undefined reference to `udbg_init_debug_lpar' Similarly for HVSI.
