blk: Ensure users for current->bio_list can see the full list.

commit f5fe1b51905df7cfe4fdfd85c5fb7bc5b71a094f upstream. Commit 79bd99596b73 ("blk: improve order of bio handling in generic_make_request()") changed current->bio_list so that it did not contain *all* of the queued bios, but only those submitted by the currently running make_request_fn. There are two places which walk the list and requeue selected bios, and others that check if the list is empty. These are no longer correct. So redefine current->bio_list to point to an array of two lists, which contain all queued bios, and adjust various code to test or walk both lists. commit e1defc4ff0cf57aca6c5e3ff99fa503f5943c1f1 "block: Do away with the notion of hardsect_size" removed the notion of "hardware sector size" from the kernel in favor of logical block size, but references remain in comments and documentation. block: rename bio bi_rw to bi_opf

Since commit 63a4cc24867d, bio->bi_rw contains flags in the lower portion and the op code in the higher portions. This means that old code that relies on manually setting bi_rw is most likely going to be broken. Instead of letting that brokeness linger, rename the member, to force old and out-of-tree code to break at compile time instead of at runtime. No intended functional changes in this commit. block: add missing group association in bio-cloning functions

When a bio is cloned, the newly created bio must be associated with the same blkcg as the original bio (if BLK_CGROUP is enabled). If this operation is not performed, then the new bio is not associated with any group, and the group of the current task is returned when the group of the bio is requested. Depending on the cloning frequency, this may cause a large percentage of the bios belonging to a given group to be treated as if belonging to other groups (in most cases as if belonging to the root group). The expected group isolation may thereby be broken. This commit adds the missing association in bio-cloning functions. This patch converts the simple bi_rw use cases in the block, drivers, mm and fs code to set/get the bio operation using bio_set_op_attrs/bio_op These should be simple one or two liner cases, so I just did them in one patch.
