Calling dev_set_name with a single paramter causes it to be handled as a format string. Many callers are passing potentially dynamic string content, so use "%s" in those cases to avoid any potential accidents, including wrappers like device_create*() and bdi_register(). [SCSI] attirbute_container: Initialize sysfs attributes with sysfs_attr_init

All of the SCSI transport classes are suddenly spitting lockdep warnings. According to Eric Biderman this is because lockdep needs static initialisers and the attribute container way of doing things end up with dynamic sysfs attributes.
