Since commit dcebd755926b ("block: use bio_for_each_bvec() to compute
multi-page bvec count"), the kernel will bug_on on the PS3 because
bio_split() is called with sectors == 0:

The problem originates from setting the segment boundary of the
request queue to -1UL. This makes get_max_segment_size() return zero
when offset is zero, whatever the max segment size. The test with
BLK_SEG_BOUNDARY_MASK fails and 'mask - (mask &amp; offset) + 1' overflows
to zero in the return statement.

Not setting the segment boundary and using the default
value (BLK_SEG_BOUNDARY_MASK) fixes the problem.

When we start sharing biovecs, keeping bi_vcnt accurate for splits is
going to be error prone - and unnecessary, if we refactor some code.

So bio_segments() has to go - but most of the existing users just needed
to know if the bio had multiple segments, which is easier - add a
bio_multiple_segments() for them.

(Two of the current uses of bio_segments() are going to go away in a
couple patches, but the current implementation of bio_segments() is
unsafe as soon as we start doing driver conversions for immutable
biovecs - so implement a dumb version for bisectability, it'll go away
in a couple patches)

More prep work for immutable biovecs - with immutable bvecs drivers
won't be able to use the biovec directly, they'll need to use helpers
that take into account bio-&gt;bi_iter.bi_bvec_done.

This updates callers for the new usage without changing the
implementation yet.

Immutable biovecs are going to require an explicit iterator. To
implement immutable bvecs, a later patch is going to add a bi_bvec_done
member to this struct; for now, this patch effectively just renames
things.

<title>Drivers: block: remove __dev* attributes.</title>

CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

This should pass "buf" to bvec_kunmap_irq() instead of "bv".  The api is
like kmap_atomic() instead of kmap().

Barrier is deemed too heavy and will soon be replaced by FLUSH/FUA
requests.  Deprecate barrier.  All REQ_HARDBARRIERs are failed with
-EOPNOTSUPP and blk_queue_ordered() is replaced with simpler
blk_queue_flush().

blk_queue_flush() takes combinations of REQ_FLUSH and FUA.  If a
device has write cache and can flush it, it should set REQ_FLUSH.  If
the device can handle FUA writes, it should also set REQ_FUA.

All blk_queue_ordered() users are converted.

* ORDERED_DRAIN is mapped to 0 which is the default value.
* ORDERED_DRAIN_FLUSH is mapped to REQ_FLUSH.
* ORDERED_DRAIN_FLUSH_FUA is mapped to REQ_FLUSH | REQ_FUA.
