Add SPDX GPL-2.0+ to all PCI files that specified the GPL and allowed either GPL version 2 or any later version. Remove the boilerplate GPL version 2 or later language, relying on the assertion in b24413180f56 ("License cleanup: add SPDX GPL-2.0 license identifier to files with no license") that the SPDX identifier may be used instead of the full boilerplate text. In preparation for unconditionally passing the struct timer_list pointer to all timer callbacks, switch to using the new timer_setup() and from_timer() to pass the timer pointer explicitly. This has the result of fixing pushbutton_helper_thread(), which was truncating the event pointer to 32 bits.
