USB: add USB_VENDOR_AND_INTERFACE_INFO() macro

commit d81a5d1956731c453b85c141458d4ff5d6cc5366 upstream. A lot of Broadcom Bluetooth devices provides vendor specific interface class and we are getting flooded by patches adding new device support. This change will help us enable support for any other Broadcom with vendor specific device that arrives in the future. commit 8815bb09af21316aeb5f8948b24ac62181670db2 upstream. On some HCDs usb_unlink_urb() can directly call the completion handler. That limits the spinlocks that can be taken in the handler to locks not held while calling usb_unlink_urb() To prevent a race with resubmission, this patch exposes usbcore's infrastructure for blocking submission, uses it and so drops the lock without causing a race in usbhid. USB: dynamically allocate usb_device children pointers instead of using a fix array

usb: Add support for indicating whether a port is removable

usb: fix number of mapped SG DMA entries

Add a new field num_mapped_sgs to struct urb so that we have a place to store the number of mapped entries and can also retain the original value of entries in num_sgs. Previously, usb_hcd_map_urb_for_dma() would overwrite this with the number of mapped entries, which would break dma_unmap_sg() because it requires the original number of entries.
