icmp.h: Remove extern from function prototypes

There are a mix of function prototypes with and without extern in the kernel sources. Standardize on not using extern for function prototypes. Function prototypes don't need to be written with extern. extern is assumed by the compiler. Its use is as unnecessary as using auto to declare automatic/local variables in a block. ipv4: fix error handling in icmp_protocol.

Now we handle icmp errors in each transport protocol's err_handler, for icmp protocols, that is ping_err. Since this handler only care of those icmp errors triggered by echo request, errors triggered by echo reply(which sent by kernel) are sliently ignored. ipv4: reduce percpu needs for icmpmsg mibs

Reading /proc/net/snmp on a machine with a lot of cpus is very expensive (can be ~88000 us). This is because ICMPMSG MIB uses 4096 bytes per cpu, and folding values for all possible cpus can read 16 Mbytes of memory. ICMP messages are not considered as fast path on a typical server, and eventually few cpus handle them anyway. We can afford an atomic operation instead of using percpu data.
