Subject: Rearrange backend startup sequence so that ShmemIndexLock can become
 an LWLock instead of a spinlock.  This hardly matters on Unix machines but
 should improve startup performance on Windows (or any port using
 EXEC_BACKEND).  Per previous discussion.
