[PATCH] sd.c
* set ->queue and ->private_data
* removed DEVICE_NR and SD_PARTITION
* switched to new methods
* switched ->open() and friends to use of ->bd_disk
* switched request handling to use of ->rq_disk
* slightly cleaned up
