[power] Cleanup pmdisk header info.
- Create struct pmdisk_info to hold image metadata (instead of struct
suspend_header).
- Put swp_entry_t to hold the beginning of the page directory, so we don't
have to reserve the last 4/8 bytes at the end of a page for it.
- Get rid of union diskpage::sh and all casts to get at it.
